- Support securing stable housing and income
- Provide information and referrals
- Support and advocacy around accessing appropriate social services, as well as mental and physical health care
Eligibility
- Ministry of Child and Family Development (MCFD) Referral, and/or
- Youth aged 13-18 and experiencing issues related to youth independence and wellbeing
Starting the Journey: Referral and Intake Process
The program accepts referrals from MCFD, as well as community referrals and self-referrals. MCFD referrals are faxed to us directly from your service provider.
New referrals are sent to the program manager for review. The manager looks at eligibility criteria and the needs of the youth to determine the best therapeutic fit between available Counsellors. If a youth is not eligible, the team will follow-up to make sure the youth is connected with the appropriate community supports.
If eligible for service, the team will follow-up to schedule a foundation meeting. In the foundation meeting, the team reviews terms of service, consent and confidentiality with the youth, as well as develops the goals of service collaboratively with the youth.
From there, the team conducts a needs assessment and works with the youth to develop an individualized service plan.
Referral Form
If you are self-referring or submitting a referral on behalf of a youth, please use the intake form below. This helps our team understand what the needs are before we reach out to set up a foundation meeting.
If you are submitting a referral form on behalf of a youth, please state clearly that the family is aware the referral is being sent and has given consent.
This form is currently undergoing maintenance. Please try again later.
